Your First Visit
First, thank you for choosing The Dental Docs as your general and cosmetic dental office. We understand the amount of trust that goes into choosing a new healthcare provider and we appreciate that you have chosen us.
Drs. Luczynski, McDonald and Haase always take extra time to communicate about exactly what their patients are wanting and needing.
The First Visit
During your first appointment, we will collect a thorough history of your dental health and you will receive a comprehensive exam to assess the health of your teeth and gums. We will then discuss your goals for care, chart any potential problems we see, and make recommendations for treatment and the next steps in your dental care.
If you are suffering from an acute issue, such as a cavity, loose crown, or other painful problem, we will also address that issue and make recommendations for treatment. Occasionally, treatment can be done the same day as the consultation. However, if you have a complex medical history that requires special care, your treatment may require a more extensive evaluation and a second appointment to provide treatment on another day.
What to Bring
- Your referral slip (if applicable)
- Your x-rays (if applicable)*
- A list of medications you are presently taking
- If you have medical or dental insurance bring your insurance card with you and complete the insurance form under “Patient Registration” link on the Forms Page. This will save time and allow us to help you process any claims.

You have a medical condition that may be of concern, for example:
Diabetes, high blood pressure, COPD, artificial heart valves and joints, rheumatic fever, etc.
You are on any medication, for example:

You require medication prior to dental cleanings, for example:
If your previous dentist has taken recent x-rays (within a year), please request that they forward them to our office. If there is not enough time for the x-rays to get to our office, you may pick them up and bring them to your first appointment. If additional images are necessary, they can be taken in our office.
Children Under 18
Since we will be discussing the child’s medical and history, allergies, and treatment plan, a parent or guardian must accompany all patients under 18 at the first visit.
